Your private health information will be kept confidential, and you will be treated with empathy, respect and professionalism.
Payment for services and co-payments are due at the time of service.
Please arrive on time for your scheduled appointment. Life happens, so if you believe that you will be late, please call in advance of your scheduled start time to inform your counselor. In some cases we will be able to adjust your start time. However, if you arrive 15 minutes after your scheduled start time, your session will likely need to be rescheduled. In this case your appointment will be counted as no-show and the no-show fee will be assessed.
If you need to reschedule your appointment, please do so within 24 hours of your scheduled appointment time. This will allow us to schedule another client that needs to be seen. Not cancelling or rescheduling within 24 hours of your appointment time will result in a no-show fee.
When assessed, the no-show fee ($150) is due at the time of your missed appointment. It will be charged to the credit card on file, and must be received before your next appointment is scheduled.
Missed group sessions are non-refundable unless cancelled and not rescheduled by the group facilitator.
Minor children of divorced or separated parents must have the signed consent of both legal guardians before mental health services are provided. When a custody order is in place, we will request a copy of the custody order specifying which parent(s) has the authority to make health care decisions for the child.
